Diploma Duplicate/Replacement
Alumni can order a duplicate or replacement diploma by completing this form. The diploma fee must be paid before the
diploma will be shipped to you. The word replacement or duplicate will appear on the diploma above the Provost's signature.
Whether replacement or duplicate are used on the diploma is determined by the type being ordered, see below.
Duplicate diplomas can be ordered in the case of additional copy, or due to a lost or damaged diploma.
Replacement diplomas can be ordered and issued in the case of official legal name change. Alumni can initiate a name change/update with the
University by completing the
Student Information Change form. Please complete the form, sign, and date and return using the options listed
on the website. Official Name changes do require a copy of the social security card with the updated/correct legal name.
Please complete all fields in this form and click the 'Pay' button and complete state treasurer's payment process.
Your request for a replacement/duplicate will be complete only when your payment has been accepted.
You can expect your replacement/duplicate diploma 6 - 8 weeks after completing this process.
